ความเป็นมา

จากการขยายตัวของสังคมและเศรษฐกิจโดยเฉพาะการเติบโตเพื่อรองรับการพัฒนาเขตเศรษฐกิจพิเศษภาคตะวันออก (EEC) ตามนโยบายของรัฐบาล ส่งผลให้มีการเพิ่มจํานวนของประชากรและ ที่ตามมาด้วยคือปัญหาสุขภาพ

โรงพยาบาลสมเด็จพระนางเจ้าสิริกิติ์ เป็นโรงพยาบาลขนาด 450 เตียงของกองทัพเรือที่ให้การดูแลทหารเรือและประชาชนในพื้นที่ภาคตะวันออก มีเป้าหมายสําคัญที่จะพัฒนาการขีดความสามารถในการรักษาระดับตติยภูมิและเป็นสถานฝึกอบรมและวิจัยทางการแพทย์ (Academic hospital) พบว่าโรคหัวใจเป็นปัญหาสุขภาพที่สําคัญและเป็นสาเหตุของการเสียชีวิตของประชาชน ที่ผ่านมาโรงพยาบาล สมเด็จพระนางเจ้าสิริกิติ์ เป็นโรงพยาบาลที่รับส่งต่อผู้ป่วยที่จําเป็นต้องได้รับการสวนหัวใจจากโรงพยาบาล ทั้งภาครัฐและเอกชน โรงพยาบาลมีแพทย์ผู้เชี่ยวชาญด้าน Cardiovascular intervention 2 คน ให้การดูแล ผู้ป่วยและทําหัตถการสวนหลอดเลือดหัวใจแก่ผู้ป่วยปีละประมาณ 760 คน ผู้ป่วยเป็นประชาชนท่ัวไปคิด เป็นร้อยละ 48 ที่เหลือคือทหารเรือและครอบครัว มีผู้ป่วยมาด้วยภาวะวิกฤตฉุกเฉินและได้รับการสวน หลอดเลือดหัวใจปีละประมาณ 270 คน นอกจากนั้น หน่วยโรคหัวใจได้พัฒนาขีดความสามารถในการ รักษาไปสู่ระดับที่สูงขึ้น ได้แก่ ความสามารถในการรักษาความผิดปกติของสรีระวิทยาทางไฟฟ้า ความผิดปกติของโครงสร้างในห้องหัวใจ การเปลี่ยนลิ้นหัวใจเอออร์ติกผ่านทางสายสวน ซึ่งเป็นโรงพยาบาลแห่งแรกในภาคตะวันออกที่สามารถทําได้ และถือ ได้ว่าเป็น Specialist Service Center

อย่างไรก็ตาม โรงพยาบาลยังมีปัญหาอุปสรรคสําคัญที่จําเป็นต้องได้รับการปรับปรุงเร่งด่วน น่ันคือการหาสถานท่ีที่เหมาะสมในการดูแลผู้ป่วยวิกฤติที่ผ่านมาผู้ป่วยกลุ่มนี้จำเป็นต้องรับการดูแลในหอผู้ป่วยวิกฤตร่วมกับผู้ป่วยอายุรกรรมโรคอื่น ๆ ซึ่งมีข้อจํากัดของจํานวนเตียงและขีดสมรรถนะของพยาบาลที่ให้การดูแลผู้ป่วยนอกจากจะส่งผลให้ผู้ป่วยไม่ได้รับการดูแลอย่างเหมาะสมแล้ว ยังเป็นอุปสรรคในการรองรับการพัฒนาศักยภาพการรักษาผู้ป่วยที่มีความซับซ้อนที่มากขึ้น

จากเหตุผลดังกล่าว หน่วยโรคหัวใจโรงพยาบาลสมเด็จพระนางเจ้าสิริกิติ์ จึงมีความจําเป็นที่จะต้องสร้างหอผู้ป่วยวิกฤติโรคหัวใจ เพื่อให้การดูแลผู้ป่วยกลุ่มดังกล่าวอย่างปลอดภัยและมีคุณภาพ

วัตถุประสงค์

เพื่อให้ผู้ป่วยโรคหัวใจและหลอดเลือดที่อยู่ในภาวะวิกฤตได้รับการดูแลอย่างเหมาะสม มีคุณภาพ และความปลอดภัยในระดับสูง
สนับสนุนให้โรงพยาบาลสมเด็จพระนางเจ้าสิริกิติ์มีศักยภาพระดับตติยภูมิ
สนับสนุนให้โรงพยาบาลเป็นสถานฝึกอบรมและวิจัยทางการแพทย์
โรงพยาบาลสมเด็จพระนางเจ้าสิริกิติ์เป็นที่พึ่งของประชาชนในพื้นที่ภาคตะวันออก ซึ่งสอดคล้องไปกับนโยบายของสาธารณสุขเขต 6 และนโยบายของกองทัพเรือ

วิธีการดําเนินการ

หอผู้ป่วยวิกฤตหัวใจและหลอดเลือดตอบสนองความเป็นเลิศทางการแพทย์ด้านโรคหัวใจ ปัจจุบันโรงพยาบาลสมเด็จพระนางเจ้าสิริกิติ์ยังไม่มีหอผู้ป่วยวิกฤตโรคหัวใจและหลอดเลือดแยกโดยเฉพาะ แต่ให้บริการรวมกับผู้ป่วยวิกฤตอายุรกรรมในหอผู้ป่วยวิกฤตอายุรกรรมซึ่งรองรับผู้ป่วย โดยรวมได้ทั้งสิ้น 8 เตียง

แนวทางการดําเนินการ ได้แก่ การสร้างหอผู้ป่วยวิกฤตหัวใจและหลอดเลือด โดยปรับปรุงจากพื้นที่ที่มีอยู่เดิมซึ่งอยู่อยู่ติดกับห้องสวนหัวใจ สามารถเชื่อมต่อกับหอผู้ป่วยวิกฤตศัลยกรรม หอผู้ป่วยวิกฤตอุบัติเหตุ และหอผู้ป่วยวิกฤตอายุรกรรม นอกจากการปรับพื้นที่แล้ว ยังจําเป็นต้องปรับปรุงระบบ ระบายอากาศให้มีความบริสุทธิ์และปลอดภัย ปรับปรุงระบบก๊าซทางการแพทย์และ Pipe Line ให้สามารถส่งก๊าซได้เพียงพอและสะดวกต่อการใช้งานของเครื่องงช่วยหายใจที่ผู้ป่วยรวมไปถึงการจัดหาเครื่องงมือและอุปกรณ์ทางการแพทย์ที่มีความแม่นยําและมีความเสถียรภาพสูง เพื่อใช้ในการวินิจฉัยและให้การรักษา ผู้ป่วยวิกฤตกลุ่มโรคหัวใจ

จํานวนผู้ป่วยที่รองรับได้

เปิดบริการช่วงแรกในเดือนก.ค. 65 – มี.ค.66 Minimum 2 เตียง (ใช้สถานที่ของห้องCathlab)
เปิดบริการช่วงถัดไปในเดือนมี.ค. 66 Maximum 4 เตียง (ห้องทั่วไป 3 ห้อง และ VIP 1 ห้อง)
ค่าใช้จ่ายเครื่องมือแพทย์ อุปกรณ์ทางการแพทย์ CCU สําหรับเปิด 4 เตียง เป็นจํานวนเงิน 21,293,000 บาท (ปัจจุบันเปิดบริการได้ 4 เตียงแต่ยังขาดอุปกรณ์การแพทย์ที่สําคัญ) และค่าใช้จ่ายรวมสถานท่ี 28,743,000 บาท
